We accept new patients who will be permanently resident in our practice area for more than three months.
Our Practice boundary covers Inverurie, Kintore and the immediate surrounding area. We also cover Rhynie and the immediate surrounding area. Please see our Practice Boundary Map for further information.
If you are living outwith Inverurie, Kintore or Rhynie please contact the Practice before completing registration forms to ensure you are living within our Practice boundary.
You will need to complete the new patient registration form (GPR) and a New Patient Health & Lifestyle Questionnaire, both can be downloaded and printed from this page.
Please bring the following documents with you:
- GPR Registration Form: To be completed in full and signed. We request proof of identity for all new patients registering. This can be in the form of a photo driving licence, passport, bus pass etc. Parent’s identification is adequate when registering children aged under 16. The GPR Registration form can be downloaded here, or is available from reception. All completed new patient registration forms should be handed in to Inverurie Health Centre for patients in Inverurie and Kintore. We cannot accept forms at Kintore Surgery. Patients living in Rhynie should hand their forms into Rhynie Surgery.
- New Patient Health & Lifestyle Questionnaire: We require information which will be held securely in your medical record. Please provide as much information as you can. The New Patient Health & Lifestyle Questionnaire can be downloaded here, or is available from reception.
Until you are fully registered with the practice, you will remain registered with your previous GP.
Until we receive confirmation of your past medical history and medication we will not be able to issue any repeat prescriptions. You should ask your previous GP for a supply.
If you are a new patient and are taking regular medication it is important you are seen by a GP before your medication runs out. This allows the GP to meet you and arrange to get your medication set up for repeat prescriptions. All new patients registering with the Practice will be offered an appointment with a Healthcare Professional within six months of registration.
Patients visiting from another area in the UK for a period of less than 3 months, and who become unwell, are entitled to be seen as a temporary resident. You will be able to make an appointment in the same way as one of our normally registered patients.
